Membership Freeze Request Form

A Membership Freeze allows you to pause your account for up to 2 consecutive months for a fee of $25. Simply purchase the Membership Freeze Request form, and we’ll send it to you to collect the dates of the freeze and signature. Once you complete and submit the form, your membership freeze will be activated.
